角色介绍：

作为HCP（医护服务计划）个案经理，您将在评估、协调和管理我们的HCP客户的护理服务方面发挥至关重要的作用。您将得到我们服务协调团队的支持。

主要职责：

进行全面评估，确定客户的个性化护理需求，并制定个性化护理计划。
协调服务的提供，包括安排家庭护理支持、护理服务、联合疗法服务和其他所需的帮助。
与多学科团队合作，确保提供符合客户目标和要求的高质量护理。
利用软件高效管理客户记录、评估和服务协议。
提供持续的个案管理，监测客户的进展，根据需要调整护理计划。
与客户、他们的家人和其他利益相关者保持定期沟通，解决任何问题，提供更新，并确保客户满意度。

经验要求：
与医疗保健、社会工作和社区护理相关的资格，例如老年护理和/或家庭和社区服务的4级证书，以及护理管理或护理方面的经验。
在家庭护理领域拥有1-3年的护理/案例管理经验。
对家庭护理计划有扎实的知识，包括评估过程、资金水平和服务提供。
熟悉老年护理质量标准（ACQS）。

技能要求：
必须具备双语技能，普通话/粤语为必备语言。
拥有澳大利亚有效驾驶执照，并可使用自己的车辆。
出色的书面和口头沟通能力，能够同客户和家人有同情心地互动。

About The Role:

As an HCP Case Manager, you will play a vital role in assessing, coordinating, and managing the care services for our HCP clients. You will be supported by our service coordination team.

Main Responsibilities:

Conducting comprehensive assessments to determine the individual needs of clients and developing personalised care plans.
Coordinating the delivery of services, including arranging for home care support, nursing care, Allied therapy services, and other required assistance.
Collaborating with a multidisciplinary team to ensure the delivery of high-quality care that meets the clients' goals and objectives.
Utilising software to manage client records, assessments, and service agreements efficiently.
Providing ongoing case management and monitoring the client's progress, making adjustments to care plans as needed.
Maintaining regular communication with clients, their families, and other stakeholders to address any concerns, provide updates, and ensure client satisfaction.



Experience:

Relevant qualifications in Healthcare, Social work, and Community Care experience e.g. Certificate 4 in Aged Care and/or Home and Community Services, Care management or Nursing.
1-3 years of experience in a care/case management role in home care sector.
Sound knowledge of home care packages, including the assessment process, funding levels, and service delivery.
Experience in the Aged Care Quality Standards (ACQS).


Skills:

Bilingual skills are essential with Mandarin/Cantonese as a compulsory language.
Valid Australian driver’s license with access to own vehicle.
Excellent communication skills, both written and verbal, with the ability to interact compassionately with clients and their families.
Strong organizational skills and the ability to manage multiple tasks simultaneously.
Exceptional time management skills to prioritise tasks and meet deadlines effectively.
Strong problem-solving and leadership skills.
Competent computer skills in Microsoft Office software and ability to learn business systems.
Flexible to changes and creative in finding efficient ways of working.
